How To Get StartedStep #1: Once you know the specific dates you need
covered, feel free to e-mail me to determine if those dates are
available. Step #2: Once I have confirmed via an e-mail that the
dates you have requested are available, it is time to click on the
link “Contract” and print out the service agreement. Step #3: The service contract needs to be fully
completed, signed and dated. Faxed back to me. The Fax
number is (830) 981-8491. Please be sure to include an after
hours phone number and e-mail address where I can reach you. Step #4: I will sign and date the service agreement
and fax back the counter signed copy to you for your records. Step #5: I will contact you by e-mail to confirm that
you have received the signed contract via Fax. Step #6: Approximately one week before the scheduled
relief period I will contact you by phone and we can discuss all
important factors concerning my relief work in your office. Step #7: Once you are back in your office, I will
contact you to update you concerning the care I have provided your
patients and to answer any questions you might have. Providing
high quality care and the continued success of your office are my
primary concerns. |
